❌ The Challenge
Patients with heart conditions often face a fragmented and overwhelming care experience. Scheduling timely appointments with specialists can be difficult, while access to medical records, test results, and prescriptions is often scattered across multiple systems. Communication between patients, doctors, and caregivers is frequently inconsistent. These gaps in coordination and access can lead to confusion, increased anxiety, missed diagnoses, medication errors, and lower engagement in long-term care management.
